Physical Examination

Students registering for Athletics must have a current physical examination form downloaded for the Athletic Trainer’s review before the student is authorized to participate in practice or tryouts.  Physicals must be dated May 1 or later and are good for 24 months.  Download the physical form by clicking the Athletic Team Registration icon and complete a family account and follow download instructions.
